Free RAID log template
Keep every risk, assumption, issue and dependency for a project in one list. Give each one a type, an owner, a priority and a due date, and the log counts how long it has been open and flags anything overdue or due this week. The summary shows what's open for each type and each person, so a status meeting takes minutes.

What's inside
- Risks, assumptions, issues and dependencies in one list, with a Type column so you can sort or filter by any of them
- An owner, priority, raised date, due date, status and next action for every item
- Days open for every item, which stops counting once you add the closed date
- A flag that works itself out: Overdue, Due this week, On track, No date or Closed
- A summary of logged, open, high priority, overdue and due-this-week items by type and by owner
- A RAID guide tab with what each type means, an example and what to record, plus room for 40 items
How to use it
Download your copy
Click "Free download" to save the .xlsx file. It opens in Excel, or in Google Sheets: open a blank sheet (sheets.new) and choose File, then Import, then Upload. The sample is a small company switching to a new CRM, so delete those rows when you're ready.
Set today's date
On the Summary tab, the first value is the date the log counts from. Type =TODAY() there so Days open and the flags stay current.
Add your items
One row per risk, assumption, issue or dependency. Give each an ID like R-01 or D-03, type the Type exactly as Risk, Assumption, Issue or Dependency, and write it as a full sentence.
Give each one an owner and a date
Name one person and pick High, Medium or Low. The due date is when the action should be done: the fix for an issue, the check for an assumption, the date you need a dependency by.
Review it every week
Walk through the open items in your weekly meeting. Update the status and next action, and look at Overdue and Due this week first.
Close, don't delete
When an item is done, set Status to Closed and type the date in Closed on. It drops out of the open counts but stays in the log as a record.
Tips
Check assumptions early
An assumption nobody checks becomes a surprise. Give each one a date to confirm it. If it turns out wrong, close it and log a new risk or issue.
Risks become issues
When a risk actually happens, close the risk and log an issue with the same subject. Put the risk's ID in the issue's next action so the history stays linked.
Name the other side of a dependency
"Vendor sets up the account" is better than "account setup". You need to know who to chase and when you need it.
Use your own team names
Change the names under Open by owner on the Summary tab. Each one counts the items in the Owner column with that exact name.
Questions
What is a RAID log?
A single list of a project's Risks, Assumptions, Issues and Dependencies. Each item has an owner, a priority, a status and a next step, and the team reviews it regularly so nothing gets missed.
What does RAID stand for?
Usually Risks, Assumptions, Issues and Dependencies. Some teams use the D for Decisions instead, or track decisions as well. You can add Decision as a type here and log them the same way, though the summary only counts the four main types.
What's the difference between a risk and an issue?
A risk might happen. An issue is happening now. When a risk turns into a real problem, close the risk and log it again as an issue.
Should I use one list or four tabs?
This template uses one list with a Type column, so every item shares the same columns and you can sort the whole log by due date or owner. Use the filter on the Type column to see one type at a time.
How are Overdue and Due this week worked out?
Each due date is compared with the date on the Summary tab. Past it is Overdue, and within 7 days is Due this week. Closed items show Closed instead.
Is it free, and does it work in Excel?
Yes. Download the .xlsx file and open it in Excel or Google Sheets. The formulas work the same in both.
Pro version
RAID Log Pro
Log every risk, assumption, issue and dependency once and see what's overdue, what's gone stale and who's carrying what, with a status report ready to print.
One log, four types, IDs that fill in
Pick Risk, Assumption, Issue or Dependency and the ID fills in (R-01, A-01, I-01, D-01). Add your own types, like Decision, on Setup.
Stale items get caught
Each priority has an update window (Critical every 3 days, Low every 30, or your own). Anything not touched in time is flagged Stale, so the log doesn't quietly go out of date.
The urgent stuff comes first
The Dashboard lists overdue, stale and due-soon items with the most urgent priority first, plus open items by type, status and age.
Assumptions get checked
Every assumption has a date to confirm it. If one proves wrong, it stays flagged until you log the risk or issue it creates and link the two.
See who's carrying what
By Owner shows each person's open items by type, with overdue and stale counts and a workload bar. Pick a name to see their list, most urgent first.
A status report ready to print
Report shows the headline numbers and each type's most urgent open items, with room for decisions and a sign-off. Print it for the weekly meeting.
